Mukwonago Catastrophic Medical Fund, Inc.

The by-laws are such that only doctors, hospitals and other medical suppliers may be paid with the funds and they are to be paid directly.

These payments are made only when all other resources have been exhausted.

Only families and patients living in the Mukwonago School District are eligible.

Anyone whose medical expenses present a hardship for their family may qualify for assistance. Beneficiaries are not required to repay the financial assistance.

In addition to financial help, families and patients are provided with information and are directed to resources other than the Catastrophic Medical Fund.

The identities of those we assist are kept confidential.

The Steering Committee, consisting of members of the community, is the governing body.

All committee members are volunteers and receive no payment.

The fund has a 501-C-3 Internal Revenue classification.
